Short answer
Upload your pest control service agreement or treatment consent form to Sign PDF and send it to the property owner or manager before the first visit. The form should list the treatment, the products' safety precautions for children and pets, the preparation the client must do, and how many follow-up visits are included.

One-off treatment or maintenance contract
Pest control paperwork falls into two shapes. A one-off service agreement covers a single job, such as a wasp nest or a mouse problem, with a set number of follow-up visits. A maintenance contract covers ongoing prevention, such as monthly rodent baiting for a restaurant or quarterly perimeter sprays for a home, with a term and a notice period.
Both should say what is guaranteed. "Free return visits within 30 days if activity continues" is a promise a client can hold you to; "guaranteed pest-free" is one you cannot keep.
Treatment consent and preparation
Before a chemical or heat treatment, the client needs to know what is being used, what they must do beforehand, and how long to stay out. A consent form signed before the visit protects both sides: the client cannot say nobody told them to move the fish tank, and you have proof you gave the warnings.
- The pest, the treatment method and the product names
- Preparation: clear cupboards, bag linen, move pets, cover aquariums, switch off pumps
- Re-entry time for people and for pets
- Known allergies, pregnancy, or asthma in the household, declared by the client
- Number of follow-up visits included
- Client's agreement that the premises will be prepared, or the visit charged
Commercial kitchens and landlords
Restaurants and food businesses often need a pest contract on file for their food safety records. The agreement should cover visit frequency, bait station locations on a site plan, and reports after each visit. Have the owner sign, not just the shift manager.
With rented homes, the landlord or agent usually pays but the tenant has to prepare the property. Send the service agreement to the landlord and a preparation and consent sheet to the tenant. Sign PDF can send both in a few minutes, and each comes back sealed with a certificate of who signed and when.
Plans and limits
A technician who signs a few new contracts a month is fine on Light, at $5 a month for 2 documents. If every job gets a consent form, you will quickly need Full at $10 a month for unlimited documents, plus templates so the consent form is ready to send.
Sign PDF does not keep chemical usage logs, schedule visits, send invoices or take payment, and we do not file anything with regulators. We get the paperwork signed and sealed so you can find it when an inspector or a client asks.
Questions people ask
Do I need a signed consent before a pest treatment?
It is good practice, and some products and jurisdictions require warnings to be given. A signed form records that the client received the safety and preparation instructions.
Can a tenant sign the preparation form and the landlord the contract?
Yes. Send them as separate documents, or put both signers on one document with their own boxes.
Should I guarantee results?
Guarantee what you control, such as free return visits within a set period. Avoid promising a pest-free property.
Can restaurants sign on a phone during service?
Yes. The owner gets an email, enters a code and signs in the browser. It takes a couple of minutes.
Can I reuse my consent form for every job?
Yes. On the Full plan, save it as a template and fill in the property and treatment each time before sending.
