What's in this salon treatment consent form

The full text. Words in brackets are filled in from the form above.

  1. 1. The parties. This consent form is between [Client name] (the Client) and [Practitioner name] of [Salon name] (the Practitioner), for a [Treatment] on [Appointment date] (the Treatment).
  2. 2. The Treatment. The Practitioner has explained what the Treatment involves, how long it takes, the expected result and how long it usually lasts. The Client has had the chance to ask questions.
  3. 3. Patch test. Where the product manufacturer recommends a patch or skin test, the Client had one on [Patch test date], at least the time before the Treatment that the manufacturer's instructions require, and reports no reaction. If the Client chose not to have a patch test, that has been recorded and the Practitioner may refuse the Treatment. A clear patch test does not rule out a reaction later.
  4. 4. Age. The Client confirms they are 18 or over. Some treatments have legal minimum ages or need a parent or guardian's consent, and the Salon will follow the rules that apply where it operates.
  5. 5. Health declarations. The Client confirms they have told the Practitioner about any of the following that apply: allergies or sensitivities, including to hair dye, tints, adhesives, latex or skincare products; a past reaction to a salon treatment; skin, scalp or eye conditions, infections, cuts or recent sunburn in the treatment area; recent treatments such as colour, perms, peels, laser or injectables; medication or skincare that makes skin more sensitive; pregnancy or breastfeeding; and any other health matter that could affect the Treatment. If unsure whether the Treatment is suitable, the Client will ask a doctor first.
  6. 6. Changes before the appointment. The Client will tell the Salon about any change to these answers before the Treatment starts, including new medication or a reaction after the patch test.
  7. 7. Risks. The Client understands that treatments can cause redness, itching, irritation, swelling or, rarely, an allergic reaction, and that results vary with hair, skin and past treatments. Colour results may differ from images or swatches. The Practitioner may stop the Treatment if there are signs of a reaction.
  8. 8. Aftercare. The Client has received aftercare advice and agrees to follow it. The Client will contact the Salon if there is any reaction, and seek medical help if symptoms are severe.
  9. 9. Price and cancellations. The price is [Price]. Appointments cancelled with less than 24 hours' notice, or missed, may be charged or lose any deposit paid.
  10. 10. Photographs. The Salon may take before and after photographs for the Client's record. They will only be used for marketing if the Client agrees separately.
  11. 11. Liability. The Client accepts the normal risks described above. Nothing in this form limits the Salon's liability for injury caused by its negligence, or anything else the law does not allow to be limited.
  12. 12. Records. The Salon will store this form and the Client's health answers securely, use them only to carry out treatments safely, and handle them in line with data protection law.
  13. 13. Governing law. This agreement is governed by the laws of [Governing law], and the courts of that place may hear any dispute about it.
  14. 14. Client confirmation. By signing, the Client confirms they have read and understood this form, that their answers are true, and that they consent to the Treatment.
  15. Signed by: the Client, the Practitioner, each with a signature, printed name and date.

When to use a salon treatment consent form

Use it before colour, bleach, perms and relaxers, lash and brow tinting, lash lifts and extensions, waxing, facials, peels and other treatments where products touch the skin, scalp or eye area. Many salons send it with the patch test appointment and have it signed before the treatment.

What the key clauses do

The form is written so the client can answer honestly in a few minutes.

  • Patch test records the date and result, or the client's choice to skip it.
  • Health declarations cover allergies, past reactions, skin conditions, recent treatments, sensitising medication and pregnancy in general terms.
  • Age records that the client is 18 or over, with a pointer to local age rules.
  • Risks explains the common reactions and that results vary.
  • Photographs separates record photos from marketing use.

How to adapt it

Use one form per treatment type, so the risks and aftercare match. Follow the product manufacturer's patch test instructions and timings. Some treatments have age limits or extra rules in some places, so check what applies to you and what your insurer requires. Keep health questions general and do not offer medical opinions.

This template is a starting point, not legal or medical advice.

Getting it signed

Fill it in, download the PDF and upload it to Sign PDF. The Client and Practitioner boxes are placed for you. Send it after the patch test so it is signed before the client sits in the chair.

Questions people ask

How long before a treatment should the patch test be?

Follow the product manufacturer's instructions. Many tints and dyes say 48 hours, but check the specific product.

What if a client refuses a patch test?

Record it on the form. Many salons will not go ahead without one, and your insurer may require it.

Do I need a new form every visit?

Many salons use a full form for the first treatment and ask clients to confirm or update their health answers at each visit, with a new patch test when products change or time has passed.

Is this template legal advice?

No. It is a plain-English starting point that a small business can adapt. Laws differ between countries and states, so have a lawyer check it before you rely on it for a high-value job or anything regulated.
