What's in this tattoo consent form

The full text. Words in brackets are filled in from the form above.

  1. 1. The parties. This consent form is between [Client name], born [Client date of birth] (the Client), and [Artist name] of [Studio name] (the Artist), for a tattoo on [Appointment date].
  2. 2. The tattoo. The Client asks the Artist to tattoo the following design: [Design], placed on the [Placement]. The Client has checked the final design, size, placement and any spelling, and approves them.
  3. 3. Age and identity. The Client confirms they are 18 or over and has shown valid photo identification. The Studio will follow any minimum age rules that apply where it operates.
  4. 4. Health declarations. The Client confirms they have told the Artist, before the tattoo starts, about any of the following that apply: allergies, including to latex, inks, metals, soaps or ointments; skin conditions, moles, scars or broken skin at or near the area; conditions that affect healing, bleeding or the immune system; medication that thins the blood or affects the skin; pregnancy or breastfeeding; a history of fainting or seizures; and any other health matter that could affect the tattoo or their safety. If unsure whether it is safe to be tattooed, the Client will ask a doctor first.
  5. 5. Condition on the day. The Client confirms they are not under the influence of alcohol or drugs, have eaten, and are not unwell. The Artist may refuse or stop the tattoo if they believe it is unsafe to continue.
  6. 6. Risks. The Client understands that tattooing breaks the skin and carries risks, including infection, allergic reaction, swelling, scarring, raised skin, and ink spreading under the skin. Colours may heal differently and fade over time, and results vary between skin types. The Artist will use sterile, single-use needles and follow the hygiene rules that apply to the Studio.
  7. 7. Permanence. The Client understands that a tattoo is permanent. Removal or cover-up can be expensive, painful and may not fully succeed.
  8. 8. Aftercare. The Client has received written aftercare instructions and agrees to follow them. Healing depends partly on aftercare, and the Artist is not responsible for problems caused by not following it. The Client will contact the Artist, and a doctor if needed, if there are signs of infection.
  9. 9. Touch-ups. One free touch-up session within three months is included if needed because of normal healing. Touch-ups needed because aftercare was not followed may be charged.
  10. 10. Price and deposit. The price is [Price and deposit]. The deposit secures the appointment and is not refundable if the Client cancels with less than 48 hours' notice or does not attend.
  11. 11. Photographs. The Artist may photograph the finished tattoo for the Studio's portfolio and social media, without showing the Client's face, unless the Client says no before the session.
  12. 12. Liability. The Client releases the Artist and Studio from claims for the normal risks described above. This does not limit liability for injury caused by the Artist's or Studio's negligence, or anything else the law does not allow to be limited.
  13. 13. Records. The Studio will keep this form and the Client's details securely for as long as local rules require, and use them only for its records and the Client's care.
  14. 14. Governing law. This agreement is governed by the laws of [Governing law], and the courts of that place may hear any dispute about it.
  15. 15. Client confirmation. By signing, the Client confirms they have read and understood this form, that their answers are true, and that they consent to the tattoo.
  16. Signed by: the Client, the Artist, each with a signature, printed name and date.

When to use a tattoo consent form

Use it for every tattoo, before the needle touches the skin. It records the age check, health questions, risks, design approval and aftercare in one signed document, and many local licensing schemes expect studios to keep a consent record like this.

Sending it when the client books lets them read it properly at home rather than skimming it at the counter.

What the key clauses do

The form is built around the questions studios and clients most often need settled.

  • Age and identity confirms the client is 18 or over and has shown photo ID.
  • Health declarations ask the client to flag allergies, skin conditions, bleeding or healing problems, blood thinners, pregnancy and fainting, without asking for their full medical history.
  • Risks and Permanence explain infection, reactions, scarring and that removal may not fully work.
  • The tattoo records design, size, placement and spelling approval.
  • Aftercare and Touch-ups link healing to following your instructions.

How to adapt it

Change the health questions to match what your local licensing authority and insurer ask for, but keep them general. Age limits, record-keeping periods and rules on tattooing minors differ between countries and local areas, so check yours. Attach your own aftercare sheet.

This template is a starting point, not legal or medical advice. It does not replace any official form your licence requires.

Getting it signed

Fill it in, download the PDF and upload it to Sign PDF. The Client and Artist boxes are placed for you. The client signs on their phone before the session and you keep the signed copy and audit trail with your records.

Questions people ask

What is the minimum age for a tattoo?

It depends on where you are. Many places set it at 18, and some do not allow tattooing minors at all, even with parental consent. Check the rules for your area.

What health questions should a tattoo consent form ask?

Keep them general: allergies, skin conditions at the site, conditions affecting healing or bleeding, blood-thinning medication, pregnancy or breastfeeding, and fainting. Your licensing authority may have its own list.

Can clients sign the consent form online?

Yes. Send it through Sign PDF when they book, and it is signed and stored before they arrive.

Is this legal or medical advice?

No. It is a starting point. Check local licensing rules and have a lawyer review it if you are unsure.
