What's in this parental consent form
The full text. Words in brackets are filled in from the form above.
- 1. Consent. I, [Parent or guardian name], give permission for [Child's name], born [Child's date of birth] (the Child), to take part in the following activity organised by [Organiser] (the Organiser) on [Dates]: [Activity or trip] (the Activity).
- 2. Parental responsibility. I confirm that I am the Child's parent or legal guardian and have the authority to give this consent.
- 3. Supervision and travel. I understand that the Child will be supervised by the Organiser's staff or volunteers, and I consent to the Child travelling by the transport the Organiser has described for the Activity.
- 4. Medical information. I have given the Organiser the following information about the Child's medical needs, allergies and medication: [Medical needs, allergies and medication]. I confirm it is accurate and I will tell the Organiser before the Activity about any change.
- 5. Medication. I consent to the Child taking the medication listed above, either by themselves or with help from a responsible adult as I have instructed. Any medication will be clearly labelled with the Child's name and dosage.
- 6. Emergency treatment. In an emergency the Organiser will try to contact me, and then [Emergency contact]. If neither of us can be reached in time, I consent to the Organiser arranging for qualified medical professionals to give the Child any emergency treatment they consider necessary. I understand the Organiser's staff are not medical professionals and will rely on first aid and the emergency services.
- 7. Fitness to take part. To the best of my knowledge, the Child is fit and well enough to take part in the Activity, apart from anything I have described above.
- 8. Behaviour. I will encourage the Child to follow the instructions of the Organiser's staff. If the Child's behaviour puts them or others at risk, the Organiser may contact me to arrange for the Child to be collected.
- 9. Photographs. My instruction on photographs and video of the Child is: [Photo consent]. I may change this at any time by telling the Organiser in writing.
- 10. Collection. Only the following people may collect the Child at the end of the Activity: [Who may collect the child]. The Organiser will not release the Child to anyone else without my direct permission.
- 11. Risks. I understand that the Activity carries some risk of injury, and that the Organiser will take reasonable care and follow its safety procedures. Nothing in this form limits the Organiser's liability for injury caused by its negligence, or anything else the law does not allow to be limited.
- 12. Personal data. The Organiser will keep this form and the Child's information securely, share medical details only with those who need them for the Child's safety, and handle them in line with data protection law.
- 13. Withdrawing consent. I may withdraw this consent at any time before or during the Activity by telling the Organiser, and I will then arrange to collect the Child.
- 14. Governing law. This agreement is governed by the laws of [Governing law], and the courts of that place may hear any dispute about it.
- Signed by: the Parent or Guardian, the Organiser, each with a signature, printed name and date.
When to use a parental consent form
Use it for school and club trips, sports tournaments, holiday clubs, residential camps, workshops and any activity where you look after someone else's child. It gathers permission, medical details, emergency contacts, photo consent and collection arrangements in one signed form.
For a simple day trip some organisers use a shorter permission slip, but a fuller form is worth it for overnight stays, travel or physical activities.
What the key clauses do
The template is built around what an organiser needs to hand on the day.
- Parental responsibility confirms the signer has the authority to consent.
- Medical information and Medication record allergies, conditions and medicines in the parent's own words.
- Emergency treatment lets you get qualified help when you cannot reach the parent, while being clear that your staff are not medical professionals.
- Photographs records the parent's choice and lets them change it.
- Collection names who may pick the child up.
How to adapt it
Describe the Activity specifically, including travel and overnight arrangements. Add a dietary needs line for residential trips. Schools and clubs often have their own safeguarding and medication policies, so make sure the form matches them, and check local rules on consent for medical treatment of children and on photographing children.
This template is a starting point, not legal advice. Ask your organisation's adviser or a lawyer to review it for higher-risk or overseas trips.
Getting it signed
Fill it in, download the PDF and upload it to Sign PDF. The Parent or Guardian and Organiser boxes are placed for you. Parents sign on their phones without an account, and you can see who has signed before the trip.
Questions people ask
Who can sign a parental consent form?
A parent or legal guardian with parental responsibility for the child. The form asks the signer to confirm this.
Does the form let staff consent to surgery?
No. It lets the organiser arrange for qualified medical professionals to give emergency treatment when the parent cannot be reached. Decisions are made by those professionals, under local law.
Is a permission slip the same thing?
A permission slip is usually a shorter version. This form adds medical details, emergency consent, photo choices and collection arrangements.
Is this template legal advice?
No. It is a plain-English starting point that a small business can adapt. Laws differ between countries and states, so have a lawyer check it before you rely on it for a high-value job or anything regulated.
